Missions & Prizes

MISSIONS
The 'Missions' tab shows you a convenient list of personalized activities for you to complete in-game, digitally, or in-person to earn tickets for your Motivv Health Studios account. Missions are recommended for you depending on your access and your individual health needs (if applicable).
 
Completing a Mission could be as easy as completing a brief survey, to completing preventive health activities such as visiting a doctor for an annual check-up that might be recommended for you based on your health needs (if applicable)*.
 
By tapping on a Mission you would like to complete, you may be able to complete the Mission immediately right in-game, such as completing a survey, or start the process to complete the Mission by helping you to schedule a doctor appointment that needs to take place in person and/or outside of our platform. Missions are worth the most tickets. Complete them to earn big!
 
PRIZES

The 'Prizes' tab shows you the available rewards that you have the opportunity to unlock with the tickets you've collected by completing available Missions, playing games and correctly answering trivia questions. The more you play, the more tickets you earn and rewards you can unlock!

You'll have the opportunity to earn up to $20 in CARE Card rewards* by collecting 10,000 tickets. The following is a breakdown of the ticket totals you will need to earn in order to unlock the rewards:

- Upon collecting 4000 Tickets you'll receive $5 on your CARE Card
- Upon collecting 7000 Tickets you'll receive an addition $5 on your CARE Card
- Upon collecting 9000 Tickets you'll receive an addition $5 on your CARE Card
- Upon collecting 10000 Tickets you'll receive an addition $5 on your CARE Card

Once you achieve the required ticket amounts to unlock your reward Motivv Health Studios will inform your health plan, which usually happens once monthly, so they can apply the reward total to your CARE Card. Additionally, you'll receive an email to let you know the reward has been earned.
